Airlines: Air Canada, AeroMexico, Midwest, Spirit, Northwest, Delta, AA, TWA, Pan Am, Southwest, United, Continental, US Air, JAL, Air France, KLM and a bunch more that I'm not recalling off the top of my head.
Equipment: 707(although I don't remember it), 717, 727, 737, 757, 767, 747,777, CRJ, ERJ, Beech 200/1900, J31/41, DC-9, DC-10, MD-11, L-1011, A319/20/21, A330, MD-80's & 90, C-130, C-5, KC-135, Cessna 150/152/172/177/182/206, Piper Archer, Arrow, Warrior, UH60, UH1, B24, ATR42, Avro RJ, Bell 206, Falcon 900, Dash 8, HS125, Fokker 100, Shorts 360, DA-20, Mooney 201, I think I've been on some variant of a Dornier but I don't remember... I'm sure I'm missing a few in here but this combines commercial flying, USAF as well as private.
The most memorable though has to be the 20 minutes I got to sweat in the seat of an F-16D. I'd trade all the rest to do that again!

I have no idea. It's only until recently (2007) that I started a log book for my flights as a passenger. Off the top of my head, here goes:
Airlines I've flown on: Cape Air, LIAT, Seaborne, Win Air, St. Baths Commuter, American, American Eagle (not the jets, Executive: ATRs), Delta, United, US Airways and various connection carriers for the above mainline carriers, Ryan International, North American, Gulf Air, Emerites, and Bmi Baby.
Planes: 732/733/738, 757, 762/763/764, 777, MD88, A319/320, A330, E170/190, CRJ200/700/900, ERJ145, BN Islander, Twin Otter, C402, DHC-8. I'm sure I've missed a few.

To answer your question Nathan, there are passenger log books out there, like the one I described. There is a lot of information that can be noted based on how in-depth you want to get. I usually log the flight time (wheels up to wheels down; I don't count taxi), a/c reg., airline, a/c type, and any notes that I can think of. Such as it the particular a/c was a special livery, first time on the type, weather delays, bad turbulence, etc.

I fall in the same category as Jack.... In 2008 alone, I flew 98,342 miles on American Airlines alone... It was a majority of domestic flights with one international flight to Belgium.
In 2009, though due to economic conditions, flying has been a lot slower... only at about 30,000 miles so far this year.....

I've flown many times but limited aircraft 727's,737's757's and the small Airbuses didn't like the Airbuses. But the most memorable was the day after getting my PPL flying right seat in a Beech Queen Air (piston version of the King Air) and flying it to landing KBDR. And 2nd sitting in my Uncles B-47 Stratojet at Pease Air Force Base shortly after the Cuban missle crisis.

Thats pretty impressive knowing all the flights, all the airlines, and all the meals too. I certainly can't make that claim. There is one particular airline meal however,that left an indelible mark on my adolescent psyche.
The year was 1972. The route was Niagra Falls to Rome. The aircraft was a 707. The meal looked like a big plump fried chicken leg. When I grabbed the leg bone to pick it up and munch on it, the little leg bone came off in my hand. Hmm..I wondered, whats up with this thing. So i grabbed my knife and fork (they issued us knives back then) and proceeded to cut into the chicken leg. When I did however, a white ,thick fluid began to ooze from the fresh wound. I leapt from my seat and almost screamed out loud.
The stew....umm, flight attendant noticed my distress and came over to calm me down. Turns out it was chicken cordon bleu. The little bone was just a prop, and the white fluid was melted swiss cheese.
Actually was'nt too bad once I got used to it.

Wow, now I cant even think to name all the destinations over the years and Im not even gonna
try. I can recall almost all the airlines and the A/C type. So here it goes.
A/C= DC-3 ( I know twice) had an engine fire one time....
DC-6 several times as a kid.
DC-8 many many times and even one time I was actually the only passenger on the plane.
707 only recall a few flights on these.
727 many many times
There was another A/C I cant recall what it was seems like it was Russian built. 35 years ago
or so.
DC-9's many times as well.
DC-10 2 times that I recall
A300 2 times
All versions I think of the MD80's many times
717 I think 2 times ( Airtran)
757 not sure how many times
L1011 a few times
SAAB 340 Not sure how many times
ATR 42 Not sure here either seems like a lot

Ill take a shot. Here goes.
Time period is 1978 - 2008
Aircraft: 727-200, 737-200/300/400/700/800, 757-200, 767-300ER/400ER, CRJ-100/200, Airbus A319/320, Mcdonnell Douglas DC-10, Lockheed L1011, MD 88/90 Bonus: i didnt fly these but Had the experience of sitting in the cockpit of a fighter jet which I cannot remember, and an F-14 Tomcat at an airshow.
Routes: Bermuda to Atlanta over 45 times on Delta 737-700/800, 757, 767, L1011 flown once on First class 767-300, Bermuda to NY LGA Delta 737-800 once, BDA to JFK American DC -10(once), American 767-200 First class once, Bda to Boston Delta 737-800, BDA to Charlotte USAir A320, A319, BDA to Newark cant remember the stats on this one. BDA to Orlando on British Airways 767 once. ATL to LAX and back over 15 times, LAX to Las Vegas, LAX to BOS, ATL to Greenville Spartansburg numerous times, ATL to CLT, LGA to GSP on a small CRJ-100 which was very tense. GSP to CLT, JFK to MIA, JFK to San Juan, And numerous other flights that i cant think of. My next big trip will be Bermuda to London on British Airways 777-200ER hopefully! Sorry for the long post but I had to lick my chops on this one!

Well, my list may be smaller, but here goes (that I can remember)

Aircraft: 737-7 or 8, 737-800 (may be the same) 757-200, 767-300, 767-400, CRJ-200, MD-88, and that's all I remember. Never been on, or care to be on, a prop aircraft or 380. I might have been on an Airbus a couple of times way back in about 2000 or 2001 when I went to Italy, in which case I've been on Alitalia. Doubt it though. I definantly don't have a log, otherwise I'd be able to list all the ones since I was born.
