This is more than likely happening because SB is looking for these files in the last place that SB was operated in. The best thing to do is remove the program completely and complete a fresh install. The ICAO_CSL.csv file should be placed in your main SB folder and any other subfolder that you add to the program. As in my case, with Windows XP HE and SB I was getting the same errors. So installed the ICAO_CSL.csv file in a subfolder I created within my main SB folder labeled Flight Plans. Turns out that when I would go to file a flight plan, being as I was looking in my Flight Plans folder and the ICAO file was not there, SB would give the error message that the file was not found. So if you have any subfolders, copy the ICAO file to each folder and you should be fine.

I have the ICAO_CSL.csv files in both places.
In regards to te other error you are receiving, if you're using a shortcut say from your desktop or start menu, right click on the shortcut and select properties, the target location should point to the SB .exe file where you have it located inside your main SB folder. The 'Start In' field should point to your main SB folder.
Target: "C:\Program Files\SquawkBox\squawkbox.exe"
Start In: "C:\Program Files\SquawkBox"
